Overview
Yale Community Health Care is a multimodal service provider, with four co-located service programs.
Our oldest program is the Community Health Care Van (CHCV) a mobile medical unit, delivering accessible care and linkage for individuals who experience challenges accessing traditional systems of care, and who have high health related social needs. The CHCV takes a harm reduction focus for all medical care provided, with syringe services provided simultaneously at both our mobile and office locations. The CHCV routinely screens for HIV/HCV/STIs, provides services related to substance use disorders (including Suboxone), wound care, and PrEP. We work alongside Federally Qualified Health Centers in New Haven, as well as the broader Yale New Haven Hospital system. We are a “bridge to care” ensuring that patients receive comprehensive services while we assist them to establish a primary medical home.
Our syringe services program the New Haven Syringe Services Program (NHSSP) is committed to providing stigma free access to harm reduction supplies and healthcare. We utilize drop-in exchange services for safer use supplies, unmarked minivan for home deliveries of SSP supplies and have harm reduction vending units that dispense Narcan, sterile needles, wound care supplies, and safer sex kits for after hours or discrete access. Additionally we offer real time FTIR drug testing –allowing our clients to test their substances for potentially dangerous additives in the drug supply (such as fentanyl and xylazine).
Our other co-located services programs include Mental Health Services, and the Mother and Infant Mobile Program.

For appointments on the CHCV:
The CHCV is a walk-in clinic! Visits are first come, first serve when the van is in the field. You are also welcome to call ahead to make an appointment on the van.
